Thank you for using SAIAB's Loan Request Form. Please furnish us with all required information.

1. Do you know which specimens you would like to borrow? (Query the specimen database)
   
   
2. Return the completed Biomaterial Transfer Agreement and a brief research proposal
   to Collections via e-mail or fax +27 (0)46 622 2403.
   Please be sure to include all required information when submitting the request and additional
   documentation.
   
   
3. Contact the collections coordinator with any additional queries.

Please enter SAIAB catalogue numbers in the boxes provided, or type taxa in the relevant Note fields.
